Salem Facility Set-Up Request Form

For information on using Salem's facilities and to schedule building use, please contact Ron Carlson, Facility Manager, 651-633-7515. Once the date and location of your event are approved, please submit a factility use request form. Please complete this form at least two weeks prior to your event, even if your set-up is minimal.

Fees may apply for non-Salem events. Please contact Ron for fee details.
